It is a surgery of millimeters, and a very small alteration will often have a profound effect on how the nose balances the face. A rhinoplasty requires extensive surgical planning, and few other operations have a greater number of variables and individual differences between patients than rhinoplasty. What is a Rhinoplasty?
A rhinoplasty is a comprehensive approach to treating a number of perceived flaws in the appearance of one’s nose. A rhinoplasty typically involves multiple small changes, leading to a natural and beautiful result. The goal is not to change your appearance so much that your friends can’t recognize you, but to enhance your natural features, and ensure your nose fits in with your facial structure and overall presence. It will treat and correct:
- A dorsal hump on top of the nose
- A wide nose
- A boxy or bulbous tip
- A droopy tip
- Flaring nostrils
A rhinoplasty will not create a “new nose,” just a refined, aesthetically pleasing nose that lives in harmony with the rest of your facial features.
Who is a Candidate for a Rhinoplasty?
Individuals unhappy with one or more features of their nose who would like to correct this surgically are candidates for a rhinoplasty. You may be a good candidate for a rhinoplasty if:
- You feel that your nose is out of balance with your other facial features
- You feel that the features of your nose are social or career obstacles
- You look in the mirror or at photos and always notice on imperfections in your nose
In addition, good candidates for a rhinoplasty include:
- Healthy individuals who do not have a life-threatening illness or medical conditions that can impair healing
- Non-smokers (smoking inhibits wound healing)
- Individuals with a positive outlook and realistic expectations regarding the results of rhinoplasty surgery

A rhinoplasty with Doctor Jason Petrungaro will be performed under general anesthesia. It is an involved procedure that will take anywhere from 3-4 hours. The surgery is done on an outpatient basis, and patients go home the same day.
The majority of rhinoplasties are performed using an ‘open’ technique. An open procedure involves making an incision across the columella (the skin bridge that separates the nostrils) to gain access to the structures being modified. This incision then extends into each nostril on the inside of the nose. The incisions are only a few millimeters long, heal very well, and are very inconspicuous because they are below and inside the nose. Following surgery the nose is taped and usually splinted. Nasal packing is determined on a case-by-case basis.
Once the incisions are made on the nose, the surgical modifications are performed. This typically includes reducing a large hump, straightening a crooked nose, reducing the width of the nose via osteotomies (breaking the nasal bones), and making the tip less boxy or bulbous.
At the end of the procedure the incisions are closed with very fine stitches, some of which may dissolve, and some of which will need to be removed in the clinic. As mentioned, the nose is taped and a hard nasal splint is placed over the top of the nose to protect the surgical site during the first week.

As discussed earlier, a rhinoplasty is a major surgical procedure, and it will require you to be committed to following all of Doctor Petrungaro’s postoperative instructions very carefully. Pain is typically very well-controlled using oral prescription pain medications.
You will be sent home with a gauze pad secured under your nose, and you can expect a small amount of drainage from the nose for the first 1-2 days. You will be given very specific instructions on how to care for the surgical site. You will see Doctor Petrungaro in his office 1-2 days following your rhinoplasty to make sure you are feeling well and everything is progressing nicely. Showering is allowed 24-48 hours after surgery.
After your surgery it is completely normal to experience facial swelling and bruising, especially in cases when the nasal bones are broken. This will resolve over 7-10 days. It will help to rest and sleep with your back and head elevated in order to help reduce this swelling. You must refrain from all strenuous activity and avoid bending forward at the waist.
Again, a small amount of drainage is to be expected the first 1-2 days. Doctor Petrungaro will be sure to go over any type of drainage or extreme swelling that would require you to call him and let him know.
You will need to take at least one week off work and rest comfortably at home. No strenuous activity is allowed for 6 weeks, and no contact sports are allowed for 8 weeks. During the first few weeks it is still important to be up and walk around, as this will help reduce swelling. After 6 weeks Doctor Petrungaro will guide you through restarting your typical fitness routines and continue to see you regularly in the office for postoperative visits.

You will be able to see results once the initial dressing and splint are removed, but be prepared to remain patient as it does take time for your swelling to resolve and for the incisions to fully heal.
The results of rhinoplasty surgery will be long-lasting. While initial swelling subsides within a few weeks, it may take up to a year for your new nasal contour to fully refine. During this time you may notice gradual changes in the appearance of your nose as it refines to a more long-term outcome.
